I will discuss Mr D, a 65 year old man with Type 2 diabetes and CKD G4 A3, who suffers an acute coronary event complicated by heart failure. We will explore whether any of his co-morbidities might have been preventable, how we could have achieved earlier CKD diagnosis and more effective management, the benefits of prompt glycaemic control and use of newer medications to reduce his risks of co-morbidities. We will also explore the role of lifestyle changes and how he might have taken control of his own health at an earlier stage. You will have the opportunity to discuss and debate with colleagues the role of education and self-empowerment in people with CKD, how you would optimise his BP at different stages in his CKD journey, and the potential role for SGLT2 inhibitors and other drugs in his care. We can undertake searches and audits to reassure us we are following guidelines and help us improve the quality of care we deliver. We will discuss and explore simple CKD-related searches with examples from a busy inner city practice.
