The prevalence of Heart Failure in primary care is frequently under reported below the expected national rate (1.2%). Patients’ diagnoses are being missed and mis-coded on a daily basis throughout the UK which prevents appropriate education, care and medication optimisation from being achieved. Ultimately, this impacts upon the patient’s quality of life and longevity.
Through the use of robust clinical audit, heart failure registers can be successfully validated, coded correctly and those patients with heart failure who have not been coded as such are discovered and managed appropriately by the clinical team.
